Assist-Life Stages, Transition and Support

Specialist Support Coordination

Specialist Support Coordination is designed for participants with more complex needs who may require a higher level of support to coordinate services, manage risks, and build a stable support network. We work collaboratively with participants, families, providers, and key supports to help create clearer pathways forward.
- Complex Support Planning. We help identify priorities, coordinate services, and support practical planning when needs, risks, or circumstances are more complex.
- Provider and Network Coordination. We work with providers, families, health professionals, and other supports to improve communication and reduce gaps in service delivery.
- Review and Problem Solving. We monitor support arrangements, respond to changing needs, and help resolve barriers so participants can work toward their goals with greater confidence.
